Creating Case Plans and Case Notes

Adding a Goal

1. From the Clients Client Profile click on the Case Plans Tab.

                  (1)   Use the pencil icon to edit existing goal

                  (2)   Click the trash icon to delete existing goal

                  (3)   Click the add goal button to ad a new goal to the client’s profile

                  (4)   You may client the notes icon to view all case notes associated with the listed goal

                  (5)   a. click the add new file attachment button to add documentation relevant to the clients goals.
                        b.  click the paper clip icon associated with the listed goal to attached relevant documentation to that goal.

                  (6)   Click the print case plan button to print the clients entire case plan

                  (7)   Exit clients profile


2. Click Add Goal. (3) The screen will refresh and display the Case Plan –Goal screen. (See Figure 2 & Figure 3)

3. Complete each field in the Goal Data section (See Figure 3) NOTE all fields that are bold with an asterisk* are required fields to set a goal:
      a. Provider: Select your default provide for which the client and client goal will be associated with by clicking My Provider (1). Alternatively click the Search button to search for another provider.
      b. Case Manager (2):Select the name of the case manager that is working with the client.
      c.  Date Goal was Set (3): Enter the appropriate date, which is self-populated with the current date. The date must be in the mm/dd/yyyy format.
      d. Classification (4): Select the applicable goal classification from the Classification picklist.
      e. Type (5): Select the corresponding goal type from the second Type picklist.
      f. Target Date (6): Enter the intended completion date for the stated goal. The date must be in the mm/dd/yyyy format.
      g. Overall Status (7): Select the option that best describes the progress toward the stated goal.
      h. If Closed, Outcome (8): there may be a possibility that the case plan is entered, and the goal may be met on the same day, and it is being entered in one step.
      i. If Partially Complete, Percent Complete (9): Select the total percentage of the goal that is complete from this field.

4. Complete each field in the follow up section: (See Figure 1-17)

            a. Projected Follow-up Date: If there will be a follow-up on this goal, enter the date you anticipate the follow-up will take place or select from one of the auto-fill icons. (1) The date must be in the mm/dd/yyyy                   format.
            b. Follow Up User: This is the user who is assigned to follow up with the client. A picklist of available users appears based on the provider. Click My Provider to select your default provider for which the user                   list will be associated (or click Search to find another provider). (2) Select the user from the pick list. (3)
            c. Follow Up Made: Select the status from the picklist. (4)
            d. Completed Follow Up Date: Enter the date the follow-up was completed or select from one of the auto-fill icons. (5) The date must be in the mm/dd/yyyy format.
            e. Outcome at Follow Up: Select from the options in the picklist. (6)

  5. Click Add Goal.  The screen will refresh and display a summary of your goal. Additionally, three new sections will appear near the bottom of the screen. (See Figure 5)

Case Notes

Once you are in the case plan section of the Client Profile screen, you can add case notes. Case notes can be added to a goal after the goal has been added to the system. Scroll down to the Case Note section and follow the steps below. (See Figure 6)

1.      Click Add Case Note. (1) The Case Note Data window will display. (See Figure 4-7)

2.      Complete the following fields; NOTE all fields that are bold with an asterisk* are required fields to open a case note:

a.      Provider: By default, the Provider associated with the goal will display; however, the provider can be changed by clicking Choose Provider. (1)

b.      Case Manager (2):Select the name of the case manager that is working with the client.

c.      Note Date (3):By default, the Note Date field is populated with the current date and time; however, you may change the date here. The date must be entered in mm/dd/yyyy format.

d.      Note(4): Enter the Case Note into the Note field.

3. Click Save Case Note. The Case Note Data window will close. The new case note will be listed in the Case Notes section on the Goals screen
