HUD VASH Exit Workflow


MSHMIS VASH EXIT WORKFLOW 

Following a clients exit from the HUD VASH program you will need to follow the steps below and exit the client accordingly. 

The process begins with the following steps:

  1. EDA (Enter Data As) into your HUD-VASH provider page, if not already defaulted to that provider bin.

To EDA into your HUD-VASH page

  1. Click on the Enter Data As link located in the top right corner of the screen 
  2. Search for your HUD-VASH Page
  3. Click the green plus (+) sign to select that provider
  4. Click Exit















  1. Select backdate mode, if not entering data in real time. 

To enter backdate mode, 

  1. Click on the Back Date link located under the Enter Data As link in the upper right-hand corner of your screen 
  2. Enter the date the data was captured, if not today’s date.
  3. Click Set Back Date 





Once you EDA into the correct bin and are in Back Date Mode, if necessary, you will proceed to the clients profile and go to the entry/exit tab. Click on the pencil icon for the client’s exit date that is in accordance with their project start date:


A box will show up and you will have to input the client’s ‘Reason for Leaving’ by using the drop-down menu provided and if you choose other, please put the reason in the Notes section. Then input the client’s ‘Destination’ by using the drop-down menu provided. If selecting ‘Other’, please specify (See Discharge Destination Guidance for additional information). Press Save & Continue at the bottom of the sub assessment.

To complete an Exit Assessment, ensure all data has been carried over from the Entry Assessment and Interim Reviews, and that all information appears correct.

  1. Go through and update the Disabilities, Health Insurance, Income, and Non-Cash Benefits sub assessments to what they are receiving at the point of exit Filling in this information is the same as ., (DO NOT ADD OR UPDATE TO WHAT IS ALREADY PUT IN THE SUB ASSESSMENTS. END DATE THE DATA ROW AND ADD A NEW SOURCE WITH THE NEW INFORMATION) 
  2. Answer Domestic Violence questions accordingly 
  3. Case management exit reason: use drop down menu to confirm what reason they exited for
  4. Update contact information if necessary
  5. Update Housing Voucher information, employment status, and general health status if needed.
  6. Save and Exit

Next steps after Exit Assessment

    1. Navigate to the Case manager tab and put an end date on the case manager that was working with the client.
    1. Next go to the Case Plans tab and enter in the final case note under the VBNL Permanent Housing Plan goal. (Do not close out the goal).
