Street Outreach Navigation HMIS Process Guide

This document provides the steps required to properly enter Detroit’s Coordinated Entry System data elements for Outreach Navigation services. Navigation intake begins after the standard HMIS Intake Process for Outreach Providers has been followed (as determined by the funding source and existing workflow) and after the client is engaged in services and has expressed interest in receiving navigation. --- i.e. assessment is complete, electronic ROI, in place, Date of Engagement recorded in the HMIS.
