VBNL JOB AIDE: ES/ HCHV VBNL ASSESSMENT WORKFLOW ADDENDUM

NOTE: Please accept the referral in HMIS ShelterPoint before starting the intake assessment and Detroit BNL assessment. Please refer to the Running and Receiving Referral Reports Job aide to either accept, cancel, or decline referrals.

Following your normal entry of data upon a client’s entry into an ES/HCHV program you will also complete the Detroit VBNL Assessment (You can navigate to the Detroit VBNL on the client’s entry assessment page example below). 




Once in the Assessment make sure all necessary information is filled out in the veteran CE questions and the VBNL referral questions (if all the questions are not answered or filled out refer to the VBNL coordinator or CAM staff accordingly)

Now that you have verified the referral information, you will complete the questions that pertain to ES/HCHV providers for VBNL updates. 


  1. In the case of GPD projects you will choose the Veteran Designated Shelter bed type.

  1. Last is updating the Current Living Situation as needed for each client. (If you are accepting a referral on the same day then ask the client if this is correct and move on).



Once you complete these two sections, you have done what you must do for your section of the VBNL assessment.

After you have completed both the MSHMIS and Detroit VBNL assessments, you can navigate to the Case Manager tab in the client’s profile and add in the client’s case manager information.

Next you will navigate to the case plans tab, and you will add case notes to the existing VBNL Permanent Housing Plan goal already inputted by the VBNL coordinator.

Last you will click on the notebook icon and the window will pop up to add case note and you will add in a new case note for the client. 


All notes need to be input in detail every 2 weeks. (This case note/goal does not get closed it stays open if the client is on the VBNL) 

Please review that all steps have been completed before to make sure all data is input correctly and completely.
